ASSIGNMENT FORM

In accordance with the Privacy Policy and Terms & Conditions of Services provided by Flycop Ltd, legal entity code [Replace with legal entity code of Flycop Ltd], registered address at [Replace with registered address of Flycop Ltd], [Replace with city and country of registration] (“Flycop”), webpage [Replace with Flycop’s webpage], which were confirmed by the Client after he/she got acquainted with them, by this assignment form (“Assignment Form”) the Client assigns to Flycop full ownership and legal title to his/her monetary claim pursuant to Regulation (EC) No 261/2004 of the European Parliament and of the Council of 11 February 2004 establishing common rules on compensation and assistance to passengers in the event of denied boarding and of cancellation or long delay of flights, and repealing Regulation (EEC) No 295/91 or under any other international or national regulation applicable in respect of the Client in another country establishing common rules on compensation and assistance to passengers in the event of denied boarding and of cancellation or long delay of flights, including all the amounts in relation to the above specified flight as well as taxes and other amounts, when passengers are not able to fly or their flight was disrupted and any other monetary compensation for lost or damaged baggage (“Assignment”).

The client is aware that by signing this assignment form, he or she is giving up the right to communicate directly with the aircraft operating carrier and to get paid by them.

In the event that the validity of this Assignment Form is ruled invalid in the specific country (jurisdiction), this agreement will be regarded as a service agreement, under which Flycop will manage the Client’s Assignment and will arrange and fund the Assignment’s collection as well as handle all associated tasks, such as but not restricted to:

• coordinating and funding the client’s legal representation before all relevant authorities and third parties regarding the Assignment; coordinating and funding the acquisition of all necessary information and the filing of complaints to the appropriate courts or administrative bodies in charge of upholding the rights of air travellers; arranging, funding, and carrying out all forms of talks in addition to other legal actions, both extrajudicial and judicial; obtaining payment for the Assignment on the client’s behalf, collecting it, and settling the dispute with the client in line with the terms and conditions listed on Flycop’s website www.flycop.com. communicating both orally and in writing with the flight operating airline about all matters pertaining to the Assignment, as well as doing any additional tasks that are associated with it.

• Organizing and financing obtention of every type of information required as well as submission of complaints to the respective courts or administrative bodies responsible for the enforcement of air passenger rights;

• Organizing, financing and conducting every type of negotiations as well as other legal – judicial and extrajudicial – measures;

• Collecting and receiving payments in relation to the Assignment on the Client’s behalf and settle with the Client in accordance with the Terms & Conditions of Services provided in Flycop’s webpage www.flycop.com

• Conducting verbal and written communication with the flight operating carrier on all issues regarding the Assignment and carrying out all other related actions.
